Affordable, Individualized "Hands On Training and Care Planning"

1st Visit (Initial Assessment): 

Includes Therapeutic Recreation Assessment of the client and

2 useful tips/interventions 

2nd Visit: 

Therapeutic Recreation Session with the client and caregiver (usually a companion or aide). Written, comprehensive Therapeutic Recreation Assessment following the visit (this is mailed within the week). 

**Must agree to 1st AND 2nd Visit to receive written assessment** 

3rd Visit:

Educational "Hands On" Activity Session with the caregiver. Review of the Therapeutic Recreation Assessment which includes goals should family agree to follow up monthly visits, as well as recommendations of appropriate activities, interventions and strategies. 

*Observational/Educational sessions are also available for
mealtime, bathing and 
other ADL's.* 

4th Visit:

Wrap up session including education to family, caregiver and all providing care. A structured Care Plan and Daily Activity Schedule (which includes Agency Care Plan if applicable), suggested supplies and materials, as well as community resources for client, caregiver and family.
